Roles & Responsibilities

• Provide compassionate, hands-on care to residents, assisting with activities of daily living (bathing, dressing, grooming)
• Monitor and record vital signs and report changes to the nursing team promptly
• Support safe patient transfers and mobility, using proper body mechanics and assistive devices
• Maintain accurate, timely documentation in resident charts and care plans
• Observe and follow infection control practices and facility policies to keep residents safe
• Communicate effectively and empathetically with residents, families, and the interdisciplinary team to support person-centered care

Qualifications

• Current Licensed Nursing Assistant (LNA) credential (T-LNA classification listed; VT/NH eligibility noted)
• Ability to provide reliable transportation — own vehicle required (public transportation in the area is limited)
• Willingness to complete facility orientation and any required competency validation for the role
• Comfort working a day shift schedule and collaborating within a team-based care model

Knowledge & Skills

• Strong hands-on resident care skills and knowledge of ADLs
• Vital sign measurement and basic clinical observation
• Clear, compassionate communication and teamwork
• Basic charting and documentation practices
• Understanding of infection control and safety protocols
• Ability to follow care plans and escalate changes appropriately
