Roles & Responsibilities

• Perform comprehensive assessments and create individualized care plans that honor patient goals
• Provide skilled pain and symptom management consistent with each hospice plan of care
• Educate and support patients and families about disease processes, self-care, and end-of-life choices
• Collaborate closely with physicians, social workers, chaplains, and volunteers to deliver coordinated care
• Document accurately and promptly in compliance with state, federal, and organizational policies
• Participate in the on-call rotation and respond promptly to patient needs
• Maintain a compassionate presence for patients and families during difficult times

Qualifications

• Current Registered Nurse (RN) license in Utah or valid compact state RN license
• Minimum of 3 years nursing experience, ideally in hospice, home health, or palliative care
• Current BLS certification
• Valid driver license and reliable transportation required
• Ability to respond within 45 minutes while on call and willingness to work holidays
• Strong clinical assessment, communication, interpersonal, and organizational skills
• Experience with Hospicesoft or Careficient EMR is preferred

Education

• Associate's or Bachelor's degree in Nursing (ASN/BSN) preferred; active RN license required

Knowledge & Skills

• Clinical assessment and care planning for terminally ill patients
• Pain and symptom management in a hospice setting
• Patient and family education around end-of-life care
• Skilled documentation and regulatory compliance
• Interdisciplinary team collaboration
• Familiarity with Hospicesoft/Careficient or other hospice EMRs
• Compassionate communication and crisis support
