Location: Prescott, Arizona

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, weekends every 3-4 weeks if at all

Hours per Week: 40

Candidate Pay Rate: $50 per hour

Roles & Responsibilities

Complete comprehensive social work assessments to understand each patient's unique needs
Conduct mental health assessments and provide critical support during vulnerable moments
Coordinate discharge planning to ensure smooth transitions for patients and families
Assist with Title 36 petitions and connect patients with vital community resources
Support individuals facing homelessness, substance abuse, and complex social determinants of health
Provide maternal child assessments and ensure appropriate reporting to DCS and APS when needed

Qualifications

Active LMSW or LCSW license in Arizona
Experience with Cerner EMR preferred
Strong assessment and care coordination skills
Compassionate approach to supporting diverse patient populations

Knowledge & Skills

Social work assessments and mental health evaluations
Discharge planning and care coordination
Understanding of homelessness, substance abuse, and social determinants of health
Maternal child assessments and mandated reporting procedures
Comfort working across inpatient, emergency, ICU, and family birthing settings
