Social Worker – UHC Staffing

Location: Prescott, Arizona

Start Date: 06-29-2026

End Date: 09-28-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, weekends every 3-4 weeks if at all

Hours per Week: 40

Candidate Pay Rate: $50 per hour

Roles & Responsibilities

Complete comprehensive social work assessments for diverse patient populations
Conduct mental health assessments and support patients in crisis
Coordinate discharge planning to ensure smooth transitions of care
Assist with Title 36 petitions and connect patients with vital community resources
Address homelessness, substance abuse, and social determinants of health
Provide maternal-child assessments and support families during critical moments
Collaborate with DCS and APS as needed to protect vulnerable populations

Qualifications

Active LMSW or LCSW license in Arizona
Comfortable working across inpatient, emergency, ICU, and family birthing settings
Experience with Cerner EMR preferred
Strong assessment and care coordination skills

Knowledge & Skills

Social work assessment and discharge planning
Mental health evaluation and crisis intervention
Navigating community resources for homelessness and substance abuse
Maternal-child and family support services
Mandatory reporting for DCS and APS cases
