Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Yolo County
Assess and evaluate each patient's therapeutic needs and develop personalized care plans
Guide patients and families through exercises, mobility training, and use of therapeutic devices
Monitor progress and communicate changes in condition to physicians promptly
Collaborate with home health aides and caregivers to support each patient's rehabilitation goals
Document clinical notes and progress summaries that help drive better patient outcomes

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits
Two supervisory references from your modality (one within 12 months, one within 3 years)

Knowledge & Skills

Home health physical therapy assessment and treatment planning
Patient and family education on therapeutic exercises and equipment
Understanding of Medicare, Medi-Cal, and Title 22 regulations
Ability to work independently while staying connected with the care team
Comfortable traveling between patient homes throughout Yolo County
