Hours per Week: 40

Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Coordinate care for patients in need of mental health support
Conduct 5150 evaluations and ensure patient safety
Collaborate with interdisciplinary team to develop discharge plans
Provide psychosocial assessments and interventions
Advocate for patients and families
Document patient progress and maintain compliance with regulations

Qualifications

Active LCSW license in California
Current BLS certification
5150 certification in California (must be obtained prior to start)
Prior experience with Dignity Health or CommonSpirit facilities
Minimum 1 year of experience in social work or care coordination
Ability to complete 5150 training and pass exam with 80%

Knowledge & Skills

Knowledge of California 5150 laws and procedures
Strong care coordination and case management skills
Excellent communication and collaboration abilities
Proficiency in electronic health records
Understanding of community resources and discharge planning
