Physical Therapist - Home Health

Location: Missoula, Montana

Start Date: 06-22-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating on-call, weekends, and holidays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Evaluate patients in their home setting and develop individualized care plans
Implement therapeutic interventions to improve mobility, strength, and independence
Coordinate care with physicians, nurses, and other disciplines for holistic treatment
Educate patients and families on home exercise programs and safety
Document all skilled services accurately and timely
Supervise Physical Therapist Assistants as needed
Adjust treatment plans based on patient progress and re-evaluations

Qualifications

Current Physical Therapist license in the state of Montana (or ability to obtain)
Valid driver's license and appropriate auto insurance
Minimum 2 years of previous PT experience preferred
BLS/BCLS certification required
Willingness to travel to patient homes in Missoula, Ravalli, and Lake Counties

Education

Bachelor's degree or higher from an accredited Physical Therapy program

Knowledge & Skills

Expertise in home health rehabilitation and patient education
Strong communication and documentation skills
Ability to work independently and as part of a multidisciplinary team
Familiarity with EMR systems (training provided if needed)
Commitment to providing compassionate, patient-centered care
