Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating weekends

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct patient evaluations and develop individualized treatment plans
Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and daily visits
Provide skilled physical therapy services to patients in their homes
Educate patients and families on therapeutic exercises and home safety
Collaborate with interdisciplinary team for coordinated care
Document patient progress and outcomes accurately

Qualifications

Active Physical Therapist license in Kentucky
BLS certification required
Minimum 1 year of home health experience preferred but not required
No new grads preferred

Education

Doctorate or Master's degree in Physical Therapy from an accredited program

Knowledge & Skills

Knowledge of home health regulations and OASIS documentation
Strong clinical assessment and treatment skills
Excellent communication and patient education abilities
Compassionate and patient-centered approach
