Roles & Responsibilities

Conduct thorough clinical assessments, screenings, diagnoses, and treatment planning for patients from birth to elders
Deliver brief therapy and continuing care to support families and individuals
Lead discharge planning and facilitate patient care conferences for seamless transitions to ALF or transitional placements
Provide expert care coordination, client advocacy, and skill-building with hospital teams and community partners
Offer crisis intervention, chemical dependency support, mental health referrals, and Power of Attorney assistance
Maintain precise, compliant documentation including progress notes, treatment plans, and reports to meet all standards

Qualifications

Masters Degree in Social Work, Psychology, Counseling or related discipline
Licensed as or eligible for Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT)
Valid State of Alaska Driver's License (or obtainable within 6 months)
Must be 21 years of age
Clinical Competency required within 3 months of hire

Education

Masters Degree in Social Work, Psychology, Counseling or related discipline from an accredited institution

Knowledge & Skills

Discharge planning and interviewing techniques
Crisis intervention and ethical/legal management
Clinical assessment, treatment planning, and brief therapy
Care coordination, client advocacy, and case management
Documentation per accreditation, state, and federal standards
