Roles & Responsibilities

• Conduct thorough screening, assessment, diagnosis, treatment planning, and brief therapy across the lifespan
• Lead and coordinate discharge planning for ALF and transitional placements to ensure safe transitions of care
• Provide care coordination and client advocacy, working closely with hospital teams and community partners
• Deliver clinical support for chemical dependency and mental health needs and make appropriate referrals
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and hospital billing standards
• Facilitate or co-facilitate patient care conferences and participate actively in multidisciplinary team meetings
• Assist with provider enrollment documentation and complete assigned billing duties
• Participate in quality management trainings and help integrate best practices into clinical services

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution
• Licensed (or in process of licensure) as LCSW, LPC, or LMFT
• Valid Alaska driver’s license or ability to obtain within six months of hire
• Must be at least 21 years of age
• Clinical competency assessment (SEARHC) required within 3 months of hire and every three years

Education

• Master's Degree in Social Work, Psychology, Counseling, or related discipline (accredited institution)

Knowledge & Skills

• Discharge planning and care coordination across the continuum of care
• Clinical assessment, screening, diagnosis, and treatment planning
• Brief therapy and continuing care for clients from birth through elder
• Crisis intervention and interviewing techniques
• Thorough documentation practices compliant with accreditation and billing standards
• Experience with chemical dependency and mental health supports
• Cultural competency and experience serving tribal health populations
• Ability to collaborate with multidisciplinary teams and community partners
• Familiarity with provider enrollment and quality management processes
