Roles & Responsibilities

• Provide evaluation and evidence-based therapy services to residents with speech, language, cognitive-communication, and swallowing needs
• Develop individualized treatment plans that set meaningful, measurable goals
• Collaborate with nursing, therapy colleagues, and family members to coordinate care and support resident outcomes
• Document evaluations, progress notes, and discharge summaries accurately and on time
• Educate residents, families, and staff on communication strategies and safe swallowing practices
• Participate in interdisciplinary team meetings and contribute to care planning

Qualifications

• ASHA Certificate of Clinical Competence (CCC-SLP) required
• 1-3 years of clinical experience preferred (SNF/rehabilitation experience a plus)
• Active or eligible for Tennessee state SLP license
• Comfortable with some floating between units or coverage needs
• Strong clinical documentation and interpersonal communication skills

Knowledge & Skills

• Skilled in assessment and treatment of adult dysphagia and cognitive-communication disorders
• Familiarity with SNF/rehabilitation workflows and Medicare documentation expectations
• Ability to create concise, compliant clinical documentation
• Collaborative approach to working within interdisciplinary teams
• Strong time management and caseload prioritization skills
