Roles & Responsibilities

• Conduct comprehensive speech-language evaluations to identify patient needs
• Deliver daily treatments focused on functional communication and swallowing
• Plan and manage discharges, coordinating with nursing and care teams for smooth transitions
• Document therapy sessions and progress in compliance with facility and CRG policies
• Collaborate with interdisciplinary team members to optimize patient outcomes
• Maintain productivity standards (therapist productivity requirement is 90%) and follow CRG policies and procedures

Qualifications

• Active SLP credential/license (state licensure or eligibility) required
• 1-3 years of clinical experience preferred (as listed in the order)
• Prior SNF/skilled nursing facility experience strongly preferred
• Ability to be present on-site to receive guaranteed hours and work within facility dress code
• Willingness to follow CRG policies and meet productivity expectations

Knowledge & Skills

• Proven ability to perform evaluations, daily treatments, and manage discharges
• Strong documentation and clinical charting skills
• Excellent communication and teamwork skills for working within an interdisciplinary rehab team
• Familiarity with SNF workflows and regulatory/documentation requirements
• Time management and caseload prioritization to meet productivity goals
