Contract Type: Contract

Shift Type: 8-hour shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, with every other weekend rotation

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop and manage daily patient lists for the palliative care team
Review patient charts and assess individual needs
Initiate goals of care discussions and educate patients on disease process
Facilitate advance directives and POLST documentation
Hold family meetings and provide end-of-life support and education
Attend daily rounds and collaborate with physicians and nurse practitioners

Qualifications

2-5 years of recent experience in palliative care, hospice, ICU, or oncology
Strong acute care clinical thinking and communication skills
Flexibility and excellent organizational abilities
A genuine desire to serve patients, families, and team members
Current BLS certification (required)
ELNEC certification preferred

Knowledge & Skills

Acute care clinical thinking
Palliative and end-of-life care principles
Cerner electronic medical record system
Effective communication and family meeting facilitation
Goal setting and advance care planning
