Location: Carmichael, California

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Contract

Shift Type: 8-hour shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, up to every other weekend

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop and manage a daily patient list, reviewing charts and assessing patient needs
Initiate goals of care conversations with patients and families
Educate on disease process, advance directives, and POLST forms
Facilitate family meetings and provide end-of-life support
Attend daily rounds with the interdisciplinary palliative care team

Qualifications

2-5 years of recent acute care experience in palliative care, hospice, ICU, or oncology
Active BLS certification required; ELNEC certification preferred
Strong clinical thinking, communication, and organizational skills
Compassionate, patient-centered approach with a desire to serve others

Knowledge & Skills

Palliative and hospice care principles
Acute care clinical assessment
Cerner electronic medical record proficiency
Advanced care planning and end-of-life communication
Ability to work collaboratively with a multidisciplinary team
