Contract Type: Travel Contract

Shift Type: 8-hour shifts, 5 days a week; flexible Monday-Friday or Tuesday-Saturday; holiday requirements (Thanksgiving/Christmas)

Hours per Week: 40

Candidate Pay Rate: $77 per hour

Roles & Responsibilities

Provide skilled nursing care to patients recently discharged from the hospital, focusing on CHF, COPD, wounds, and PICC line management
Conduct start of care, recertification, resumption of care, and routine visits based on point system
Educate patients and families on disease management and equipment use
Collaborate with home health aides and other team members for comprehensive care
Maintain accurate documentation in HomeCare HomeBase (HCHB)

Qualifications

Current RN license in California
BLS certification required
Valid driver's license and auto insurance
HCHB (HomeCare HomeBase) experience preferred

Knowledge & Skills

Expertise in CHF and COPD management
Wound care and wound vac management
IV therapy and PICC line care
Strong patient education skills
Ability to manage 5 visits per day with varying acuity
