Shift Type: Monday-Friday (5x8), 7:30 AM - 4:00 PM, Day shift; every other weekend required
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Coordinate patient discharges and transitions of care to ensure safe, timely movement to the next level of care
- Conduct needs assessments and arrange durable medical equipment and community resources
- Develop and document individualized plans of care using EPIC with accuracy and clarity
- Perform utilization review and case management activities in line with CMS and facility regulations
- Collaborate with interdisciplinary teams including social services to resolve barriers to discharge
- Educate patients and families about discharge plans, follow-up, and available supports
- Manage an active caseload with attention to quality, efficiency, and patient-centered outcomes
- Float between hospital areas (including Colby and Pacific campuses) as clinical needs require
Qualifications
- Active RN license (or MSW for social work candidates) — RN licensure required for RN role
- BLS certification required
- Minimum 1 year of acute care experience, with discharge planning/care coordination experience
- Experience with Epic charting required
- Comfort working in acute hospital settings such as Med-Surg, ED, ICU or LLOS strongly preferred
- Pending licenses accepted; please include all RTO information at time of submission
- Willingness to work every other weekend and float to different units as needed
Education
- Registered Nurse (RN) with active state license or Master of Social Work (MSW) for social work applicants
- BLS certification required
Knowledge & Skills
- Case management and utilization review workflows
- Discharge planning and care coordination best practices
- CMS regulations and regulatory compliance awareness
- Ability to perform needs assessments and order/coordinate DME
- Strong documentation skills in Epic
- Interdisciplinary communication and patient/family education
- Ability to manage varying patient ratios and prioritize effectively in a busy acute-care environment
