Roles & Responsibilities

• Provide direct patient care under the direction of a licensed Physical Therapist, delivering compassionate, evidence-based treatments
• Record daily treatment notes and weekly progress notes in accordance with PT Board requirements
• Assist in maintaining a safe, clean, and well-equipped treatment area to support quality care
• Participate in patient care and rehabilitation conferences to coordinate multidisciplinary plans
• Support the nursing department by training Restorative Aides and contributing to caregiver education
• Communicate patient progress, problems, and plans clearly with supervisors and the healthcare team
• Document treatment changes per facility policy and ensure accurate EMR entries
• Instruct patients, families, and nursing staff on maintenance programs to promote successful discharge outcomes
• Report equipment issues promptly to ensure therapy tools remain in good working order

Qualifications

• Licensed Physical Therapy Assistant in the state of California (current and in good standing)
• Proficiency with electronic medical records such as Casamba and PointClickCare
• Demonstrated ability to provide patient-centered care and follow physician treatment plans
• Strong communication skills and ability to train caregivers and restorative staff
• Willingness to participate in facility in-service training programs
• Must meet continuing education requirements as required by the state board

Education

• Licensed Physical Therapy Assistant per state board requirements

Knowledge & Skills

• Patient care and rehabilitation therapy best practices
• Accurate clinical documentation and progress note writing
• Caregiver training and patient/family education techniques
• Familiarity with Casamba and PointClickCare EMR systems
• Basic equipment maintenance and treatment area upkeep
• Ability to collaborate with interdisciplinary teams and participate in care conferences
