Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes throughout greater Sacramento county
- Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
- Help patients regain mobility and independence with personalized treatment plans
- Collaborate with interdisciplinary teams to deliver exceptional home health care
- Document patient progress and communicate effectively with supervisors and care teams
Qualifications
- Active Physical Therapy Assistant license in California
- Minimum experience preferred, new grads considered in exceptional cases
- Strong clinical skills across multiple patient populations
- Excellent communication and time management abilities
- Valid driver's license and reliable transportation
Knowledge & Skills
- Home health care experience
- Patient assessment and treatment planning
- Geriatric, orthopedic, and neurological rehabilitation
- Documentation and compliance standards
- Mileage tracking and reimbursement processes
