Roles & Responsibilities
- Deliver personalized physical therapy care to patients in the comfort of their homes
- Design and implement treatment plans that help patients regain mobility and independence
- Collaborate with physicians and care teams to ensure seamless, patient-centered care
- Educate patients and families on exercises and techniques to support recovery
- Document progress and adjust care plans to meet each patient's unique goals
Qualifications
- Current California Physical Therapist license required
- BLS certification required
- Home health experience is a plus but not mandatory
- Strong communication and time-management skills
Knowledge & Skills
- Patient assessment and individualized care planning
- Therapeutic exercise and mobility training
- Home safety evaluation and adaptive equipment recommendations
- Electronic documentation and care coordination
