Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes across Tehama County
Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
Develop personalized treatment plans for orthopedic joint replacements and geriatric patients with multiple comorbidities
Complete approximately 5 meaningful patient visits per day using your clinical expertise
Document care efficiently using the HomeCare HomeBase EMR system on your provided tablet
Collaborate with the intake RN and manager to ensure seamless patient care coordination

Qualifications

Current Physical Therapy license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Strong clinical skills in orthopedic and geriatric care

Knowledge & Skills

Physical Therapy expertise in home health settings
Experience with orthopedic joint replacements and geriatric patients with CHF and COPD
Proficiency with HomeCare HomeBase EMR system
Excellent time management and independent work skills
Strong communication and patient education abilities
