Roles & Responsibilities

• Assess patient mobility, strength, and functional needs to build personalized care plans
• Deliver evidence-based therapeutic interventions including gait training and therapeutic exercise
• Collaborate with nurses, physicians, and rehab aides to coordinate comprehensive patient care
• Document clinical progress clearly and timely in the electronic medical record
• Educate patients and families on home programs and fall prevention to maximize independence
• Participate in team meetings and discharge planning to ensure smooth transitions

Qualifications

• Licensed Physical Therapist in Pennsylvania or eligible to obtain PA licensure
• Graduate of an accredited Physical Therapy program; DPT preferred
• Current CPR/BLS certification
• Experience in inpatient rehabilitation or acute care preferred
• Strong interpersonal and communication skills with a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy accepted with eligibility for licensure

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, balance and functional mobility interventions
• Comfortable with manual therapy techniques and adaptive equipment recommendations
• Proficient with clinical documentation and electronic medical records
• Ability to assess and progress patients toward measurable functional goals
• Team collaboration, problem solving, and clear patient education
