Roles & Responsibilities

Evaluate and treat patients with mobility and functional impairments to improve independence
Develop and implement individualized treatment plans tailored to each patient
Provide therapeutic exercises, gait training, and manual therapy to advance recovery
Monitor patient progress and modify treatment plans as needed for best outcomes
Maintain accurate and timely documentation to support continuity of care
Collaborate with interdisciplinary healthcare teams to coordinate patient-centered care

Qualifications

Active California PT license (required)
Current BLS certification
Previous SNF or rehab experience preferred
Strong clinical judgment and excellent communication skills
Available to start ASAP for this 13-week full-time assignment

Knowledge & Skills

Patient evaluation and functional assessment
Treatment planning and individualized care design
Therapeutic exercises and gait training techniques
Manual therapy skills and hands-on interventions
Accurate clinical documentation and charting
Interdisciplinary collaboration and care coordination
