Roles & Responsibilities

• Assess patients' functional status and mobility to create individualized treatment plans
• Deliver therapeutic interventions and hands-on treatment to improve strength, balance, and independence
• Monitor progress, adjust plans as needed, and document outcomes clearly and promptly
• Collaborate with nurses, case managers, and families to support safe discharges and continuity of care
• Educate patients and caregivers on home exercise programs and fall prevention strategies
• Support facility quality initiatives and contribute to a positive, patient-centered environment

Qualifications

• Valid Physical Therapist license in California or eligible for state licensure
• BLS/CPR certification (current)
• Previous experience in inpatient rehabilitation or post-acute care preferred
• Strong clinical reasoning, assessment, and manual therapy skills
• Excellent communication and teamwork skills; compassionate patient-centered approach

Education

• Degree in Physical Therapy (DPT preferred; Master's or Bachelor's in Physical Therapy acceptable where applicable)

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, and balance interventions
• Ability to perform thorough functional assessments and create measurable goals
• Proficient with clinical documentation and electronic health record systems
• Knowledge of safety protocols, mobility equipment, and adaptive devices
• Comfortable collaborating within an interdisciplinary care team
• Time management and organizational skills to manage caseload efficiently
