Roles & Responsibilities

• Assess patients' functional mobility and develop individualized treatment plans to reduce pain and improve independence
• Deliver therapeutic interventions focused on mobility training, fall risk reduction, and pain management
• Document evaluations, progress notes, daily notes and discharge summaries accurately and promptly
• Coordinate discharge planning and communicate with interdisciplinary team members to support patient transitions
• Handle insurance communications including approvals/denials and relevant phone conversations
• Maintain a clean, safe treatment environment and assist with light facility tasks as needed

Qualifications

• Licensed Physical Therapist in Wisconsin or eligible for licensure
• Doctor of Physical Therapy (DPT) preferred or equivalent degree in physical therapy
• Strong clinical documentation skills, including experience with progress notes and discharge documentation
• Comfortable communicating with patients, providers, insurance companies, and rehab team members by phone and in person
• Professionalism, reliability, and the ability to work independently within a small team

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in Physical Therapy or equivalent considered

• Patient assessment and evaluation
