Location: Santa Maria, California

Start Date: 06-18-2026

End Date: 09-17-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services to patients in their homes, helping them regain independence and improve quality of life
Develop and implement individualized treatment plans based on patient needs and goals
Collaborate with interdisciplinary care team including OTs, SLPs, and nursing staff
Document patient progress using Oasis and Cerner EMR systems
Educate patients and families on home exercise programs and safety strategies
Cover a designated geographical region, traveling to patient homes as scheduled

Qualifications

Active Physical Therapist license in the state of California
Minimum 1 year of experience as a Physical Therapist
Experience with Oasis documentation required
Home health experience preferred but not required
Current BLS certification from the American Heart Association
Ability to work independently and manage a flexible schedule

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Proficiency in Oasis and Cerner EMR systems
Excellent communication and patient education skills
Time management and organizational skills to coordinate home visits
Knowledge of orthopedic, neurological, and general rehabilitation principles
Ability to adapt interventions to diverse home environments
