Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes across assigned geographic regions
- Evaluate and treat a diverse caseload including orthopedic conditions, strokes, joint replacements, and spinal injuries
- Complete OASIS documentation to meet government-mandated requirements
- Collaborate with interdisciplinary teams including OT and Speech Therapy colleagues
- Provide compassionate care that helps patients regain independence and improve quality of life
Qualifications
- Active Physical Therapist license in California
- BLS certification required
- Minimum one year of experience working with PT/OT/Speech populations
- Experience with OASIS documentation required
- Home health setting experience preferred but not mandatory
Knowledge & Skills
- Strong understanding of OASIS documentation requirements
- Proficiency with Cerner EMR system
- Excellent clinical assessment and treatment planning abilities
- Comfortable working independently in home-based care environments
- Effective communication and patient education skills
