Roles & Responsibilities
- Assess and evaluate patients' therapeutic and functional status to create personalized care plans
- Provide hands-on physical therapy treatments that help patients regain mobility and independence
- Educate patients and families on therapeutic exercises and equipment use for continued progress at home
- Document patient progress and communicate updates to physicians to ensure coordinated care
- Collaborate with home health aides and caregivers to support comprehensive rehabilitation goals
- Identify additional patient needs and connect families with appropriate community resources
Qualifications
- Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong oral and written communication skills with excellent organization abilities
- Valid Physical Therapy license for California practice
- Ability to work independently while thriving in a collaborative healthcare team
Knowledge & Skills
- Physical Therapy assessment and treatment techniques
- Patient evaluation for muscle strength, mobility, gait, and range of motion
- Home environment assessment and safety evaluation
- Care plan development and goal setting
- Patient and family education strategies
- Documentation and clinical reporting skills
