Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes throughout Tehama County
- Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
- Develop personalized treatment plans for orthopedic joint replacements and geriatric patients with multiple comorbidities
- Complete approximately 5 meaningful patient visits per day using your clinical expertise
- Document care efficiently using the HomeCare HomeBase EMR system on your provided tablet
- Collaborate with the intake RN and manager to ensure seamless patient care coordination
Qualifications
- Current Physical Therapy license in California
- BLS certification required
- Home Health experience preferred but will consider travelers with less than 2 years experience if they have Home Health background
- Valid driver's license and reliable transportation for home visits
Knowledge & Skills
- Physical Therapy expertise in home health settings
- Experience with HomeCare HomeBase EMR system
- Strong assessment and care planning skills for orthopedic and geriatric patients
- Excellent communication and patient education abilities
- Comfortable with independent practice and decision-making
