Contract Type: Travel Contract

Shift Type: Saturday-Sunday, 7:00 AM - 11:00 PM

Hours per Week: 40

Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Conduct in-person participant assessments to determine the right level of care
Deliver hands-on wound care, IV hydration, lab draws, and nursing assessments during home visits
Respond promptly to calls from providers and support staff, prioritizing needs with care
Document all interactions thoroughly and follow up as needed to ensure continuity
Educate participants, families, and facility staff with compassion and clarity
Collaborate with providers, pharmacies, hospitals, and community partners for seamless support
Initiate emergency treatment when necessary and keep leaders informed
Champion quality improvement by completing incident reports and participating in ongoing training

Qualifications

Active Michigan RN license required
Associate degree in Nursing required; Bachelor's preferred
At least one year of experience with frail, elderly, or long-term care populations
Home care experience preferred
Wound care experience preferred
Current CPR certification by AHA or willingness to obtain
Reliable transportation, valid Michigan driver's license, and car insurance

Education

Associate degree in Nursing required
Bachelor's degree in Nursing preferred

EMR documentation proficiency
Strong communication and collaboration across care teams
Ability to adapt to changing workloads and schedules
Participant-centered care approach
