Roles & Responsibilities

Provide skilled physical therapy evaluations and develop individualized treatment plans
Deliver evidence-based interventions to improve mobility, function, and independence
Collaborate with interdisciplinary team members to coordinate patient-centered care
Document patient progress accurately and in a timely manner
Educate patients and families on home exercise programs and strategies for prevention
Contribute to quality improvement efforts and maintain compliance with facility standards
