Roles & Responsibilities

• Assist licensed physical therapists in delivering high-quality rehabilitation care to residents
• Implement individualized therapeutic exercise programs and gait training to improve mobility
• Monitor patient progress and document treatment notes clearly and accurately
• Collaborate with nursing and interdisciplinary teams to coordinate care plans and ensure safety
• Educate patients and families on home exercise programs, fall prevention and adaptive strategies
• Maintain equipment, ensure treatment areas are safe and infection control standards are followed
• Support outcome-driven care by tracking goals and communicating changes to the supervising therapist

Qualifications

• Licensed Physical Therapist Assistant (PTA) or eligible for state licensure
• Graduation from an accredited PTA program
• Current Basic Life Support (BLS) / CPR certification preferred or willing to obtain
• Comfort working in long-term care and skilled nursing environments
• Strong interpersonal skills and a patient-centered approach to care

Education

• Associate degree from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Knowledge of therapeutic exercise principles, gait training and mobilization techniques
• Skilled in patient assessment, documentation and goal-oriented treatment planning
• Ability to use common rehab equipment and assistive devices safely
• Excellent communication, teamwork and time management skills
• Familiarity with infection control and safety protocols in a clinical setting
