Roles & Responsibilities

• Evaluate patients' functional abilities and needs with clear, person-centered assessments.
• Develop individualized occupational therapy treatment plans that target meaningful daily activities.
• Implement therapeutic interventions to improve independence with activities of daily living (ADLs).
• Monitor patient progress, adjust plans as needed, and document outcomes thoroughly.
• Collaborate with nursing, rehab disciplines, and families to coordinate excellent, holistic care.
• Train patients and caregivers on adaptive strategies and safe use of assistive equipment.
• Contribute to a positive facility culture by participating in team meetings and quality initiatives.

Qualifications

• Licensed Occupational Therapist (OTR/L) in Virginia or eligible for Virginia licensure.
• Experience in a SNF or long-term care setting preferred; familiarity with skilled nursing workflows is a plus.
• Strong documentation skills and comfort using electronic medical records.
• Compassionate communicator who can collaborate with interdisciplinary teams and families.
• Active professional liability insurance as required by facility policies (if applicable).

Education

• Bachelor's or Master's degree in Occupational Therapy (or accredited occupational therapy program).

Knowledge & Skills

• Skilled in conducting functional assessments and developing measurable goals.
• Proficient with ADL re-training, transfer and mobility training, and adaptive equipment recommendations.
• Knowledgeable about rehabilitation techniques for older adults and medically complex patients.
• Strong clinical reasoning, time management, and prioritization skills.
• Effective verbal and written communication for patient education and documentation.
