Roles & Responsibilities

• Assess and evaluate patients’ functional abilities and develop individualized treatment plans
• Deliver skilled occupational therapy interventions focused on ADLs, mobility, and adaptive equipment
• Collaborate closely with nurses, PTs, SLPs and the interdisciplinary team to optimize patient outcomes
• Document patient progress clearly and timely to support interdisciplinary care and discharge planning
• Educate patients and families on strategies and equipment to promote independence and safety
• Participate in weekly team meetings and contribute to continuous quality improvement efforts

Qualifications

• Active Michigan Occupational Therapist license or eligible to obtain state licensure
• OTR credential preferred (or equivalent state credential)
• Experience or strong interest in Skilled Nursing Facility / rehabilitation settings
• Current CPR/BLS certification (or willingness to obtain upon hire)
• Strong clinical assessment, treatment planning, and documentation skills
• Excellent communication and teamwork approach with patients, families and staff

Education

• Degree in Occupational Therapy (Bachelor's, Master's, or OTD) as required for state licensure

Knowledge & Skills

• Skilled in ADL training, adaptive equipment recommendations, and functional mobility
• Knowledge of SNF/rehab regulations and best practices for resident-centered care
• Competent in clinical documentation and EHR charting
• Ability to educate and motivate patients and caregivers
• Strong problem-solving and time-management skills in a fast-paced setting
• Familiarity with interdisciplinary collaboration and discharge planning
