Roles & Responsibilities

• Monitor and record vital signs accurately to ensure patient safety
• Perform medication reconciliation and coordinate pharmacy/refill requests
• Conduct telephone triage and follow-up phone calls with compassion and clarity
• Obtain patient histories, PHQ/allergy documentation, and chart thoroughly in EPIC
• Collect and label specimens (including clean-catch urine) and interpret laboratory results for clinician review
• Prepare and set up aseptic/sterile procedure trays and perform simple dressing changes
• Support fast-paced outpatient/urgent care clinic flow and float between units as needed within your skillset
• Communicate effectively with the care team to deliver high-quality family medicine and OB patient care

Qualifications

• Licensed Vocational Nurse (LVN) — current and in good standing
• Minimum 1 year of Adult Outpatient nursing experience (Family Medicine/OB experience preferred)
• Experience charting in EPIC (charting system required)
• BLS certification preferred
• Willingness to submit a statement at time of application confirming openness to a temp-to-hire opportunity (required)
• Local candidates preferred — role is focused on those within a 50-mile radius
• First-time outpatient LVNs are welcome to apply

Education

• Licensed Vocational Nurse (LVN) certification/license

Knowledge & Skills

• Proficient in vital sign monitoring and patient assessment
• Skilled in medication reconciliation and pharmacy/refill communications
• Telephone triage experience with strong patient communication
• Specimen collection and labeling, including clean-catch urine
• Aseptic technique and simple dressing changes
• Ability to interpret basic laboratory results and escalate as needed
• Comfortable working in EPIC and documenting accurately
• Experience in outpatient, urgent care, family medicine, or OB settings
