Roles & Responsibilities

Provide compassionate bedside nursing care to residents and patients
Administer medications and treatments accurately and on time
Monitor and document vital signs, changes in condition, and care provided
Collaborate with RN staff, therapists, and interdisciplinary team members
Maintain accurate patient records in the electronic medical record
Support residents with activities of daily living while preserving dignity
Communicate clearly with families and care teams about patient needs

Qualifications

Current, active LPN license (able to practice in Connecticut required)
CPR/BLS certification (or willingness to obtain upon hire)
Comfortable working in a long-term care or skilled nursing environment
Strong communication skills and ability to work as part of a team
Reliable, compassionate, and patient-centered approach to care

Education

Licensed Practical Nurse (LPN) diploma or vocational nursing certificate

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and restorative nursing principles
Patient assessment and monitoring of vital signs
Familiarity with electronic medical record (EMR) systems
Time management, prioritization, and strong interpersonal skills
