Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents
Administer medications and treatments accurately and safely
Monitor and document vital signs, observations, and resident changes
Collaborate with the care team to develop and follow individualized care plans
Communicate clearly with residents and families to ensure comfort and understanding
Maintain accurate electronic and paper charting in line with facility policies
Support a positive, team-focused environment and assist with resident activities as needed

Qualifications

Current, active LPN license in Connecticut (required)
BLS/CPR certification (or willingness to obtain upon hire)
Comfortable with medication administration, basic wound care, and resident assessments
Strong communication and teamwork skills with a resident-first mindset
Prior experience in long-term care or skilled nursing settings is a plus

Education

LPN diploma or certificate from an accredited program

Knowledge & Skills

Practical nursing skills: med administration, vital signs, basic wound care
Electronic medical record/charting familiarity
Infection control and safety best practices
Ability to prioritize and manage time during an 8-hour shift
Strong interpersonal skills for working with residents, families, and staff
