Roles & Responsibilities

• Provide compassionate, high-quality nursing care in accordance with facility policies and state regulations
• Administer medications and treatments safely and accurately
• Monitor resident health status and promptly report changes to the care team
• Document clinical observations and interventions clearly and timely
• Collaborate with nurses, therapists, and support staff to create person-centered care plans
• Support resident activities of daily living while promoting dignity and independence
• Communicate effectively with residents and families, providing updates and education as needed

Qualifications

• Active LPN license in Pennsylvania (required)
• Current BLS/CPR certification (required or obtained upon hire)
• Comfortable with medication administration and charting
• Strong communication and teamwork skills
• Prior experience in long-term care, skilled nursing, or similar settings preferred
• Able to pass facility background checks and required screenings

Education

• Vocational nursing diploma, certificate, or equivalent training required

Knowledge & Skills

• Working knowledge of basic nursing principles and clinical best practices
• Familiarity with electronic charting and accurate documentation
• Medication administration and infection control procedures
• Ability to assess patient condition and escalate concerns appropriately
• Organizational skills to manage tasks across multiple residents
