Roles & Responsibilities

• Provide compassionate direct patient care, including medication administration and wound care
• Monitor patient status and communicate changes clearly and promptly to the care team
• Document assessments, treatments and patient responses accurately and timely
• Assist with admissions, discharges and daily care planning to support continuity
• Collaborate with nurses, therapists and providers to deliver patient-centered care
• Support residents' dignity and independence through respectful, personalized care

Qualifications

• Current Licensed Practical Nurse (LPN) license — Alabama or compact eligibility recommended
• Minimum clinical experience working in long-term care or similar settings preferred
• Basic Life Support (BLS) / CPR certification (or ability to obtain prior to start)
• Up-to-date immunizations and any facility-required health screenings
• Excellent communication skills and a team-oriented attitude

Education

• Completed LPN/Vocational Nursing program and active LPN license

Knowledge & Skills

• Strong skills in medication administration and charting/documentation
• Good assessment skills and ability to recognize and escalate changes in condition
• Proficiency in basic nursing procedures common to long-term care
• Ability to work collaboratively with interdisciplinary teams
• Time management and prioritization in a busy clinical environment
