Roles & Responsibilities

• Provide compassionate bedside care to residents at Bloomfield Nursing and Rehab, ensuring dignity and comfort
• Administer medications and treatments according to policy and provider orders with accuracy and attention to detail
• Monitor, assess and document patient status, reporting changes promptly to the care team
• Assist residents with activities of daily living and support rehabilitation goals
• Collaborate with nurses, therapists and interdisciplinary staff to deliver coordinated care plans
• Maintain clear, timely electronic and paper charting to support continuity of care
• Support patient and family education, answering questions and providing reassurance

Qualifications

• Valid Licensed Practical Nurse (LPN) license in New Mexico
• Current BLS/CPR certification required
• Experience in long-term care, skilled nursing or rehab preferred
• Ability to pass facility-required background checks and health screenings
• Local candidates only (this is a local-only assignment)

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Strong clinical assessment and vital signs monitoring skills
• Medication administration competence and knowledge of infection control practices
• Skilled in wound care and basic restorative nursing principles
• Comfortable using electronic medical records and clear documentation practices
• Excellent communication and teamwork skills with residents, families and staff
