Roles & Responsibilities

• Provide compassionate, person-centered care to residents, assisting with activities of daily living
• Monitor and document resident status, including vital signs and changes in condition
• Assist with mobility, transfers and fall-prevention measures to keep residents safe
• Follow care plans and collaborate with nurses and interdisciplinary team members
• Ensure a clean, comfortable environment and support infection control practices
• Communicate clearly and empathetically with residents and their families
• Participate in shift-to-shift handoffs and accurate charting to maintain continuity of care

Qualifications

• Active Licensed Nursing Assistant (LNA) credential — NH (VT/NH designation noted in the posting)
• Current CPR/BLS certification (or willing to obtain)
• Comfortable performing hands-on resident care and working as part of a team
• Excellent communication skills and a compassionate bedside manner
• Reliable, punctual, and able to complete assigned shifts at Elm Wood Center

Education

• High school diploma or GED preferred
• Completion of a state-approved nursing assistant training program

Knowledge & Skills

• Strong understanding of activities of daily living (ADLs) and basic resident care
• Ability to take and record vital signs accurately
• Basic charting and documentation skills
• Knowledge of infection control and safety protocols
• Good observational skills to notice and report changes in resident condition
• Teamwork, time management, and the ability to prioritize resident needs
