Roles & Responsibilities

Conduct thorough psychosocial assessments to understand patient needs
Develop personalized treatment plans that make a real difference
Deliver evidence-based therapeutic interventions for patients across all ages
Collaborate with an integrated care team to provide whole-person care
Support mental and emotional well-being in a rural community setting

Qualifications

Active LISW license in Iowa
Master’s degree in Social Work from an accredited program
Minimum of 2 years of clinical experience preferred
Experience in rural or medically underserved settings is a plus

Education

Master’s degree in Social Work from an accredited program

Knowledge & Skills

Psychosocial assessments
Individualized treatment planning
Evidence-based therapeutic interventions
Collaboration with integrated care teams
Strong interpersonal and organizational skills
Excellent documentation abilities
