Roles & Responsibilities

Receive and give detailed reports on residents' conditions to ensure seamless care
Make regular rounds, assess new or ill residents, and document everything per policy
Notify nurses promptly of any changes in residents' conditions or needs
Assist with direct patient care and respond quickly to call lights
Foster strong relationships with co-workers, residents, families, and physicians
Work collaboratively as a team player across departments

Qualifications

Active California CNA License required
Good communication skills and team player attitude
Ability to float to other units as needed

Education

Certified Nursing Assistant (CNA) certification

Knowledge & Skills

Strong assessment and documentation skills
Excellent communication and teamwork abilities
Resident-focused care in SNF setting
