Roles & Responsibilities

• Assess and evaluate residents’ mobility, strength, balance and functional needs to create meaningful goals
• Design and deliver individualized treatment plans that improve safety, independence and quality of life
• Collaborate with nursing and interdisciplinary teams to coordinate care and optimize outcomes
• Document progress accurately and promptly to support continuity of care and regulatory compliance
• Educate residents and families on activity plans, fall prevention and adaptive strategies
• Advocate for resident-centered care that respects dignity and promotes recovery

Qualifications

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — or eligible for NH licensure
• PT or DPT credential (entry-level candidates and new grads welcome — any experience level accepted)
• Comfort and experience (or strong interest) working in a Skilled Nursing Facility / long-term care setting
• Strong communication and teamwork skills to partner effectively with interdisciplinary staff
• Current BLS/CPR preferred or willing to obtain (confirm requirement with facility)

Education

• Doctor of Physical Therapy (DPT) or equivalent degree in Physical Therapy
• New graduates accepted — strong clinical supervision and onboarding provided

Knowledge & Skills

• Skilled in geriatric assessment, mobility training and restorative care
• Proficient documentation and outcomes tracking for SNF care plans
• Ability to create practical, functional treatment plans tailored to resident goals
• Sound clinical judgment for safe transfers, gait training and fall-risk mitigation
• Excellent interpersonal skills for resident and family education
