Roles & Responsibilities

• Assess patients' physical function and mobility needs and establish individualized treatment plans
• Provide skilled therapeutic interventions to restore or maintain patient independence
• Collaborate closely with nursing and interdisciplinary team members to coordinate care
• Document progress and outcomes accurately and timely in the clinical record
• Educate patients, families, and staff on safety, mobility strategies, and therapeutic goals
• Adjust treatment plans based on patient response and discharge planning needs
• Participate in team meetings to ensure cohesive, patient-centered care

Qualifications

• PT or DPT credential (PT/DPT) — the listing specifically seeks PT/DPT clinicians
• Open to clinicians at all experience levels, including new graduates
• Experience or interest in Skilled Nursing Facility (SNF) practice strongly preferred
• Strong communication and teamwork skills to work within an interdisciplinary setting
• Ability to complete required documentation and meet facility compliance standards

Education

• DPT or equivalent physical therapy degree; new graduates welcome

Knowledge & Skills

• Skilled in patient assessment and developing individualized treatment plans
• Knowledge of SNF rehabilitation standards and regulatory documentation
• Ability to safely manage transfers, gait training, and assistive device recommendations
• Proficiency in communicating clinical progress to patients, families, and staff
• Comfortable working in a fast-paced, interdisciplinary long-term care environment
