Roles & Responsibilities

• Evaluate patients to develop individualized treatment plans focused on function and mobility
• Implement therapeutic interventions including exercise, manual therapy, and mobility training
• Document patient progress clearly and timely in the medical record
• Collaborate with interdisciplinary team members to coordinate care and optimize outcomes
• Educate patients and families on home programs, safety, and strategies for independence
• Monitor and adjust treatment plans based on patient response and goals
• Support quality improvement initiatives and maintain best-practice standards

Qualifications

• Active Physical Therapist license in New Jersey or eligibility to obtain licensure
• Doctor of Physical Therapy (DPT) degree preferred or equivalent clinical training
• Current BLS/CPR certification preferred
• Strong clinical assessment and documentation skills
• Comfort working in a fast-paced rehabilitation setting and collaborating with a care team

Education

• Doctor of Physical Therapy (DPT) preferred; relevant graduate degree or equivalent clinical experience considered

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and manual techniques
• Proficient with documentation and outcome measurement
• Knowledge of rehabilitation principles for adult and post-acute populations
• Excellent communication and patient education abilities
• Ability to work collaboratively with nursing and interdisciplinary teams
