Roles & Responsibilities

• Assess patients’ mobility, strength, balance and functional needs to create individualized treatment plans
• Deliver evidence-based therapeutic interventions including therapeutic exercise, gait training and manual techniques
• Collaborate with nurses, therapists and the interdisciplinary team to coordinate patient-centered care
• Educate patients and caregivers on recovery goals, home exercise programs and fall prevention strategies
• Document clinical findings and progress clearly and timely in the facility EMR
• Participate in discharge planning to ensure safe transitions and optimal outcomes

Qualifications

• Current Physical Therapist license in Pennsylvania or ability to obtain state licensure prior to start
• Professional degree in Physical Therapy (DPT, MPT or equivalent)
• BLS/CPR certification (or ability to obtain promptly)
• Experience in skilled nursing, inpatient rehab, or post-acute care preferred
• Strong interpersonal skills and ability to work collaboratively in a fast-paced care setting

Education

• Doctorate or Master’s in Physical Therapy (DPT/MPT) or equivalent clinical degree

Knowledge & Skills

• Clinical assessment and therapeutic exercise prescription
• Gait training, balance and functional mobility interventions
• Manual therapy principles and safe patient handling techniques
• Clear, efficient documentation and familiarity with EMR systems
• Patient and family education with an emphasis on functional outcomes
• Time management and prioritization in a multidisciplinary environment
