Roles & Responsibilities

• Provide compassionate direct patient care as a Licensed Practical Nurse, delivering safe and effective nursing interventions
• Administer medications and treatments accurately, following physician orders and facility protocols
• Document patient assessments, care plans and progress notes clearly and promptly
• Collaborate with RNs, physicians and interdisciplinary team members to ensure continuity of care
• Monitor patients’ conditions and escalate changes in status to the appropriate clinical staff
• Support patient and family education with kindness and clarity to promote positive outcomes

Qualifications

• Active Licensed Practical Nurse (LPN) license valid in Pennsylvania (required)
• Comfortable working evening/PM shifts and committed to the every-other-weekend schedule (required)
• Current Basic Life Support (BLS) / CPR certification preferred or willing to obtain
• Strong communication skills, clinical judgment and ability to work well within a care team

Education

• Completion of an accredited LPN / practical nursing program (diploma or certificate)

Knowledge & Skills

• Solid knowledge of fundamental nursing care, medication administration and basic assessments
• Comfortable with documentation best practices and maintaining accurate patient records
• Good clinical observation skills and the ability to prioritize patient needs
• Teamwork mindset and strong patient-centered communication
