Roles & Responsibilities

• Provide compassionate, safe patient care during overnight shifts in the Uptown Health/Rehab area
• Perform thorough patient assessments and document findings accurately in the medical record
• Administer medications and treatments according to orders and facility protocols
• Collaborate with the interdisciplinary team to develop and update individualized care plans
• Monitor patient status and escalate changes promptly to the appropriate provider
• Support patients and families with education, discharge planning, and emotional support
• Assist with admissions, transfers, and safe patient handoffs at shift change

Qualifications

• Active LPN or RN licensure (Licensed Practical Nurse or Registered Nurse) — required
• Experience or interest in post-acute care / rehabilitation settings is a plus
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Strong clinical assessment, medication administration, and documentation skills
• Reliable, team-oriented, and committed to patient-centered care

Knowledge & Skills

• Solid knowledge of clinical nursing practices and basic rehab care
• Comfortable with medication administration and patient monitoring
• Clear, professional communication with patients, families, and team members
• Good time management, prioritization, and documentation skills
• Ability to work independently overnight while collaborating with the care team
