Roles & Responsibilities

• Provide compassionate, hands-on care to residents including assistance with activities of daily living
• Assist with personal hygiene, mobility support and safe transfers to promote resident dignity and safety
• Observe and report changes in resident condition promptly to the nursing team
• Document care accurately and timely in the facility charting system
• Support infection control practices and maintain a clean, safe care environment
• Collaborate with nurses and interdisciplinary team members to deliver person-centered care
• Build trusting relationships with residents and their families through clear, respectful communication
• Participate in required trainings and help maintain high standards of care

Qualifications

• Current LNA or T-LNA certification (Licensed Nursing Assistant)
• Eligible to work in Vermont or New Hampshire as noted in the job posting
• Availability for evening shifts (3:00 PM - 11:00 PM) and every-other-weekend coverage
• Strong communication skills and the ability to work well on a team
• Recent experience in long-term care or similar setting preferred
• CPR/BLS certification preferred or willing to obtain

Education

• High school diploma or GED preferred
• Completion of an approved LNA/CNA training program

Knowledge & Skills

• Skilled in assisting with ADLs (bathing, dressing, toileting, feeding)
• Knowledge of basic infection control and safety procedures
• Comfortable with resident transfers and safe mobility techniques
• Ability to observe, record and communicate changes in condition
• Familiarity with basic charting and documentation practices
• Good time management and the ability to prioritize resident needs
