Roles & Responsibilities

• Evaluate residents thoroughly and create individualized therapeutic plans of care with clear short- and long-term goals
• Deliver skilled therapeutic interventions and progress treatment toward functional goals using appropriate modalities
• Document all evaluative and therapeutic interactions accurately and accessibly in the Electronic Medical Record
• Collaborate with interdisciplinary team members and consult with referring physicians on resident care and discharge planning
• Recommend, obtain, and instruct residents and caregivers in the use of assistive devices
• Supervise and mentor assistant-level staff and support new staff orientation to ensure high-quality care
• Provide in-service education and contribute to development of new rehabilitation programs
• Maintain infection control practices and ensure clinical areas and equipment are clean and safe

Qualifications

• Active Physical Therapist license in the state of practice (required)
• Bachelor's degree in a related field required; Master’s or Doctorate preferred
• Basic Life Support (BLS) certification preferred
• Prior experience in long-term care or skilled nursing preferred
• Strong documentation skills and familiarity with Electronic Medical Records
• Knowledge of Medicare and Medicaid reimbursement requirements
• Ability to perform essential physical duties including lifting up to 50 pounds and frequent movement
• Excellent communication, time management, and interdisciplinary teamwork skills

Education

• Bachelor's degree required; Master’s or Doctorate in Physical Therapy preferred

• Medicare/Medicaid reimbursement knowledge and compliance
